Hardin County Department of Job and Family Services is issuing a Request for Proposals (RFP) to solicit quotes from qualified providers for transportation services for Medicaid eligible individuals and all other Agency approved transportation. The NET program will assist Medicaid recipients through the County with transportation to and from medical appointments both inside and outside of Hardin County, Ohio. They will transport qualified Medicaid recipients anywhere in the state of Ohio. Services are to be provided to all consumers of all ages that are Medicaid eligible, with a variety of medical needs or disabilities, to and from Medicaid reimbursable services/appointments. Services are set to begin July 1, 2021 through June 30, 2022 with a mutual agreement that we could extend it another 2 years for a total of 3 years with no increase in rate or change in services provided. Extension would be contingent upon availability of funds, including compliance with all applicable budgetary and legal requirements and satisfactory performance by the contractor. Proposal must provide cost of transportation on way, idle time cost, and proof of appropriate insurance for liability, loss, damage, and/or related expenses incurred through provisions of services. All proposals will be evaluated, and selection based upon the criteria outlined in the RFP. HCDJFS will reserve the right to accept or reject any and all proposals, the right to waive any informalities in the proposals received, and to cancel in part or in its entirety the RFP.
